You can speed up the intake process at Mahle Chiropractic by filling out some of the paperwork in advance. Here is everything else worth knowing before your first visit at 101 Decker Dr.

- Monday: 9:00 AM to 5:00 PM
- Tuesday: 9:00 AM to 6:00 PM
- Wednesday: 9:00 AM to 5:00 PM
- Thursday: 1:00 PM to 6:00 PM
- Friday: 9:00 AM to 5:00 PM
- Saturday and Sunday: Closed
Same day appointments are available, and Spanish speaking representatives are on hand at the desk.

Two forms, ready when you call ahead
New Patient Form
The standard intake paperwork every first-time patient fills out. Call the office at (724) 656-9050 or ask the front desk to have it ready before you come in, and you will spend less of your appointment holding a clipboard.
Auto Accident Form
If you are coming in for treatment related to an auto accident injury, this is the second form the office needs. The same rule applies: call ahead or ask at the desk and it will be waiting for you.
