Speed up your visit before you arrive
You can speed up the intake process by filling out some of the paperwork in advance. New patients fill out the New Patient Form, and anyone being treated for an injury from a car accident fills out the Auto Accident Form as well. Both are covered on the new patient information page.
Students must be accompanied by a parent or legal guardian on their first visit. If you are unsure which form applies to you, call the office and the front desk will walk you through it.
